Help... build finished but voltage error [solved]

I finished my build (first timer). I have infinite resistance between terminal 12 and 22. (also between any ground point and terminal 22. I have checked, double checked and triple checked the wiring. I can't see anything wrong.

What do i do?

Triple check how you have the power transformer mounted, especially those start washers. 

How's the resistance from ground to terminal 16?

Terminal 16 is grounded. Should the transformer frame be grounded? I'm assuming the start washers and backplate washers are intended to isolate the xfmr case from ground?

You're a genius. Completely skipped the part where I need a lock washer under the bolt nearest the safety ground. Thank you so much!!!!!
